ภาพยนตร์ภาคต่อสุดระทึกที่เต็มไปด้วยความคลุ้มคลั่ง ในอดีตนักผจญภัย
กลุ่มหนึ่งได้ลงไปเผชิญกับมฤตยูร้ายในส่วนที่ลึกสุดของโลก เหลือเพียง
ซาร่าห์ คาร์เตอร์ ที่เหลือรอดชีวิตจากถ้ำลึกลับเพียงลำพัง ไม่มีใคร
สามารถล่วงรู้ได้เลยว่าเกิดอะไรขึ้นที่ถ้ำแห่งนั้นและทำไมตัวซาร่าห์ถึง
ได้เกรอะกรังไปด้วยเลือดของมนุษย์ จนกระทั่งเป็นตัวเธอเองที่ตัดสินใจ
กลับลงไปเผชิญกับนรกในถ้ำแห่งนั้นเพื่อตามหาเพื่อนของเธอที่ไม่รู้ว่า
อยู่หรือตาย ทั้งๆ ที่รู้ว่าฝันร้ายนั้นจะกลับมาหลอกหลอนเธออีกครั้ง
